Insurance

Although I do not accept insurance, your insurance carrier may pay for “out-of-network” therapists.   In such cases, I will provide you with a receipt for submission to your insurance carrier for possible reimbursement.

 

Appointments

Appointments can be scheduled by calling me directly, or sending me an email.  Should you leave a message, please be sure to leave your phone number so that I can return your call as quickly as possible.

 

Forms

Please review and sign the consent form and bring with you to your first appointment.
